Notice on How to Apply
for Excused Absences
Included in this documents will have
instructions and samples on completing the required document for excused
absences
Please check the attached file. (Application for
excused absence form and example file)
General Points
In the application form, you need to get the TA
and Department Chair Professor’s signature/stamp in the “Approved By” box at the bottom of the form.
After you upload the required documents, you
should visit the department or phone TA office to confirm the final approval
confirmation. (The RESPONSIBILITY is yours to check for the final
confirmation). All of these absence
application form deadlines are within one week of their occurrence.
All absence application form 신청/접수/반려 = uploaded/applied/rejected one week after the
date of absence cannot be accepted as attendance.
(Based on the date and time of the final change
in the info system).
ex) Absent from January 4th/ write and get
signature of absence form on January 5th/ Upload on info system on January 6th/
Get final approval on 7th of January.
Excused Absences that occur during the
semester must be within one week from the date (or the last day) of the
absent date.
During make-up week of the semester, you have the until the last
day to complete the process thus the process needs to be started on the day of
excused absence or the following day but no later than the last day of the
make-up week.
※You can apply for absence form of make-up week
only for that week.

Table 1 Type of Excused Absences
On #3 Illnesses: you are allowed 3 days per
semester total and not by course.
In principle, you need to do this immediately
and no later than the day after the affected date, visit the TA office of W19
218 to get required signature, and scan and upload it with a scanner by
yourself within a week. Mobile
scanned file will NOT be accepted.
On #3 Illnesses: the supporting evidence
documents will have 2 documents (including the medical confirmation and a
medical expenses receipt.)
*Card receipt is NOT allowed, Only official
treatment receipt given by hospital*

School Activities/Events
On # 7 to 10” Supporting evidence Document
You need to contact and inform the TA in advance
of the activity/event. It is impossible to modify/change and accept
supporting-evidence after the official event period has ended. Notification to
the TA needs to be done in advance which includes the paperwork process and
supporting evidence documents.
Please fill out the department name and subject
name as shown in the info system. Make sure to completely and accurately fill
in the required information about Department Name and Course Name for the
affected excused absence due to the events.
*How to upload a excused absence form*
Info System -> Academic Management -> Course -> Course
Management -> 공결 신청 -> Select the subject and select a date for public
decision application, and click ‘New’ -> upload the form and evidence
document and save.
※ After scanning the absence and evidence
document, only upload as a PDF file. one application (attendance
recognition request + absence statement)
*Again,
only PDF file will be accepted ( THESE types will be rejected such as zip file, HWP, DOCX, JPG, etc., you will
be rejected.)
★ Photo
of excused absence and supporting documents, will NOT be accepted which
includes using mobile apps that scans. All other methods will not be accepted
by Academic Affairs.
Finally, please make sure and check the attached
file has the correct information before you upload them.
(Completed Application Form is accepted only,
missing parts will be rejected.)
